Requires both the insured person and health care provider to sign before the insured can receive direct payment from a health care insurer when benefits have been assigned to the provider.

Applies when an insured assigns their right to receive benefits to a health care provider for care that was provided to the insured.

Defines "health care insurer" by reference to the definition in Arizona Revised Statutes section 20-3101.

Adds section 20-241 to Title 20, chapter 2, article 1 of Arizona Revised Statutes.
