Requires death certificates to record a decedent's sex to reflect their gender identity as reported by the person completing the certificate.

Allows death certificates to be amended to reflect gender identity when a document memorializing gender transition is presented, including court orders, written instructions, health care directives, clinical treatment records, or changes to birth certificates or government IDs.

If a document memorializing gender transition is not presented and the person(s) with the right to control the disposition of remains disagrees with the reported gender identity, the certificate shall reflect the gender identity as reported by that person or majority of persons.

Defines documents that memorialize gender transition as court orders for name or gender changes, written instructions from the decedent, health care directives, proof of clinical treatment, or documentation of gender marker changes on government records.

Protects health care providers from civil liability or professional disciplinary action for completing and signing medical certifications of death in good faith.
