Requires corporations, limited liability companies, and labor organizations to register and notify filing officers within one day of making contributions exceeding specified thresholds to influence candidate elections: $5,000 or more in statewide races, $2,500 or more in legislative races, and $1,000 or more in local races.

Registration must include the entity's name and address, plus the name, title, email, and phone number of the person authorizing the contribution.

Each contribution notification must include the entity name, contribution amount, recipient committee or organization, candidate name and race, and contribution date.

Requires a notarized sworn statement within five days confirming the filer had authority to make the contribution on behalf of the entity; notifications without verification are categorized as unverified or delinquent.

Establishes civil penalties up to three times the total contribution amount for failure to register or notify, and classifies knowingly false filings as class 1 misdemeanors.
