• Limits in vitro fertilization coverage to three complete cycles (fresh or frozen embryo transfers), with unlimited frozen transfers if embryos from prior retrievals are available; requires American Society for Reproductive Medicine guidelines compliance.

• Mandates coverage for fertility preservation services when medical treatments may cause iatrogenic infertility from surgery, radiation, chemotherapy, or other medical procedures.

• Requires coverage of prescription drugs for infertility diagnosis and treatment without different exclusions, limitations, deductibles, copayments, or waiting periods than other prescription drugs.
