Requires health professionals administering vaccines to provide patients (or parents/guardians of minors) with specific information before vaccination: vaccine benefits and risks, manufacturer's product insert, CDC vaccine excipient and media summary, and instructions on reporting vaccine-adverse events.

Amends department of health rules to require that any informational or educational materials developed for parents and guardians about immunizations must include all information required for informed consent under the new vaccine informed consent law.

Clarifies that pupils may not be immunized without informed consent of parent, guardian, or person in loco parentis, and specifies that pupils age 18 or emancipated may consent to immunization themselves.

Makes minor technical corrections to existing immunization law, including changing "which" to "that" and "of" to "after" in relevant sections regarding health agencies and reimbursement timelines.
