Amends Arizona Revised Statutes Section 15-873 to modify the religious exemption language for school immunizations by changing "personal beliefs" to "religious beliefs" only.

Requires parents or guardians claiming religious exemption to submit a signed statement confirming they received information about immunizations and understand the risks and benefits before refusing consent.

Maintains existing medical exemption provisions allowing physicians or registered nurse practitioners to certify when immunizations may be detrimental to a pupil's health.

Excludes non-immunized pupils from attending school during outbreak periods of communicable, immunization-preventable diseases as determined by the department of health services or local health department.
