Adds physical therapy as a covered health and medical service under the Arizona Health Care Cost Containment System (AHCCCS) program.

Physical therapy must be ordered by a primary care practitioner for the number of outpatient visits determined by the practitioner to be medically necessary.

Removes a previous subsection that allowed the director to provide additional, reduced, or modified hospitalization and medical care benefits for certain eligible persons.

Makes conforming changes to subsection lettering to account for the removal of the subsection regarding modified benefits.
