Requires health profession regulatory boards to maintain an address of record for each licensee that may be disclosed to the public, with licensees able to opt out if they designate their residential address.

Mandates that licensees required to maintain patient medical records provide the board with a telephone number or e-mail address for patients seeking access to their records.

Allows health profession regulatory boards to designate associations of licensed health professionals that may receive contact information and addresses of all regulated professionals, including those who opted out of public disclosure, but prohibits these associations from transferring or selling the information.

Permits boards to provide licensee information to the Department of Health Services or Arizona Board of Regents universities for research purposes if the data are not distributed publicly in a format containing personally identifiable information.

Modifies section 32-3801 to clarify that residential addresses and telephone numbers are confidential except as provided under the new section 32-3226.
