Amends Arizona Revised Statutes Section 15-873 regarding immunization exemptions for school attendance.

Specifies that pupils at any level or grade through twelfth grade are eligible for a religious exemption from immunization requirements based on parental personal beliefs.

Maintains medical exemptions signed by both parent/guardian and a physician or registered nurse practitioner, valid only during the duration of the medical condition that precludes immunization.

Requires pupils lacking immunization documentation to be excluded from school during outbreak periods of communicable immunization-preventable diseases as determined by the department of health services or local health department.
