Requires health professionals administering vaccines to provide patients (or parents/guardians for minors) with four specific pieces of information before vaccination: benefits and risks of each vaccine, the vaccine manufacturer's product insert, the CDC's vaccine excipient and media summary, and information on how to report vaccine-adverse events.

Mandates that any informational or educational materials developed by the Department of Health regarding immunizations must include all informed consent information required under the new vaccine consent requirements.

Specifies that a pupil may not be immunized without informed consent from the parent, guardian, or person in loco parentis, and clarifies that pupils 18 years or older or who are emancipated may consent to their own immunization.

Maintains that immunization against human papillomavirus is not required for school attendance.

Makes minor technical corrections to existing immunization statutes regarding grammar and terminology.
