Adds a new exception to Arizona's parental consent requirement for minors seeking abortion, allowing a physician to perform an abortion without parental consent if the minor receives specified pregnancy counseling and information.

Requires counseling to include explanation that information is objective and non-coercive, discussion of alternatives (carrying to term, adoption, foster care), information on prenatal and postnatal care, and information on birth control agencies.

Mandates that counseling explore involving the minor's parent or guardian in decision-making and provide opportunities for the minor to ask questions about pregnancy, abortion, childcare, and adoption.

Requires the physician or counselor providing counseling to have the minor sign a form documenting receipt of required information and counseling, with a copy provided to the attending physician.

Allows the counseling exception to apply only if the physician provides the counseling directly or refers the minor to a qualified counselor who complies with all specified requirements.
