Establishes a family caregiver grant program beginning January 1, 2020, providing grants to Arizona residents caring for qualifying family members in their homes.

Grants equal 50% of qualifying expenses (home modifications, assistive equipment/technology) up to $1,000 per qualifying family member, with annual income limits of $75,000 (single) or $150,000 (married filing jointly).

Applicants must be state residents whose family members require assistance with daily living activities as certified by a physician, nurse practitioner, or physician assistant and meet specified relationship requirements.

Awards grants on first-come, first-served basis up to $500,000 annually; grantees cannot reapply for three consecutive years; department may consult advisory council on aging for application review.

Appropriates $1,500,000 in fiscal year 2019-2020 to establish the family caregiver grant program fund and repeals the program effective June 30, 2023.
