Prohibits health insurers, HMOs, and disability insurers from removing covered diabetes medications, insulin, or diabetes test strips from formularies during the policy term unless the FDA questions safety or the manufacturer discontinues the product.

Prohibits reclassifying diabetes medications or supplies to more restrictive drug tiers or increasing cost-sharing (deductibles, copayments, coinsurance) unless a generic equivalent becomes available.

Prohibits reducing maximum coverage limits for diabetes medications and supplies during the policy term.

Allows insurers to add new diabetes medications to covered drug lists and does not restrict generic drug substitution.

Applies restrictions to health care services organizations (HMOs), disability insurers, and group or blanket disability insurers offering diabetes coverage.
