Adds section 36-675 to Arizona Revised Statutes establishing criteria for exemption from immunization requirements for employment, school attendance, or licensure.

Allows exemptions only if a vaccine meeting all six specified criteria is unavailable, including long-term cancer and fertility impact evaluations and at least one year of safety data compared to placebo or another approved vaccine.

Requires the state health department to post known vaccine injuries and disease rates on its website and conduct studies on vaccine interactions with other state-mandated vaccines.

Mandates that proven risk of permanent disability or death from the vaccine must be less than from the infection it prevents and vaccine manufacturers must have liability for design defect claims.

Permits individuals to claim exemptions for themselves or their dependents without meeting additional conditions, prohibiting state agencies from imposing extra requirements.
