Requires health insurance policies with prescription drug coverage to provide coverage for all FDA-approved contraceptive drugs and devices, including oral, implant, and injectable contraceptives, intrauterine devices, and prescription barrier methods.

Prohibits insurance companies from imposing deductibles, coinsurance, copayments, or other cost-sharing measures for contraceptive drugs, intrauterine devices, prescription barrier methods, or male sterilization procedures.

Applies to hospital and medical service corporations, health care services organizations, disability insurers, and blanket disability insurers offering group coverage.

Exempts religiously affiliated employers from contraceptive coverage requirements if they file a written affidavit with their insurer stating religious objection, but does not exclude coverage for contraceptives prescribed for non-contraceptive medical purposes.

Does not apply to individual nongroup health insurance policies or plans.
