HB 2532 Summary

• Requires the Arizona Department of Health Services to approve uniform prior authorization request forms for prescription drugs/devices/durable medical equipment and for other health care procedures by January 1, 2021.

• Mandates all providers use only the approved uniform forms and all health care services plans and utilization review agents must accept these forms starting January 1, 2022; prior authorization requests submitted after that date on non-uniform forms are invalid.

• Specifies uniform forms shall not exceed two printed pages and must meet electronic submission and acceptance requirements.

• Directs the department to consider existing forms from federal agencies, national electronic prior authorization standards, and input from stakeholders including providers, health plans, pharmacists, and pharmacy benefit managers when approving the uniform forms.

• Allows payors to continue using internet-based prior authorization systems if they are consistent with the approved uniform forms.
