State agency heads are prohibited from asking job applicants to provide their wage history.

Applicants may voluntarily provide wage history information to state agencies, and agencies may request confirmation of voluntarily disclosed wage history after making a job offer.

State agencies must develop procedures and standard forms to verify applicants' education and work history, including documented good faith efforts to contact current and previous employers.

"State agency head" includes the state agency head's designee for purposes of this statute.
