AZ HB2600

Allows adopted individuals 18 years of age or older born in Arizona to obtain a copy of their original sealed birth certificate from the state registrar upon written request, effective December 31, 2020.

Requires birth parents to submit a contact preference form indicating whether they want direct contact, contact through an intermediary, or no contact with the adopted individual, and allows updates to preferences at any time.

Mandates birth parents to acknowledge during adoption proceedings that adopted children may obtain their original birth certificate at age 18, and to submit a contact preference form to the court.

Establishes a medical history form for birth parents to complete optionally and keep sealed with the original birth certificate for the adopted individual to receive.

Appropriates $1,000,000 to the Department of Health Services for implementation and publicity of these provisions.
