Amends Arizona Revised Statutes Section 28-669 regarding accident report forms.

Requires written accident reports by investigating officers to be made on department-approved forms containing all required information unless unavailable.

Authorizes the department to modify the written accident report form in conjunction with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
