All state agencies, boards, commissions, and other administrative units must ask individuals at their initial point of service whether they or a family member have ever served in the military.

This question must be included on all application forms used by state agencies.

The requirement applies to every individual seeking services from a state agency for the first time.

The act is effective as of December 31, 2020.
