Establishes presumed consent for organ, eye, and tissue donation for all Arizona residents 18 years or older upon death, unless they have specifically refused.

Allows individuals to refuse anatomical gifts through signed documents, wills, or oral communication during terminal illness to at least two adults (one being a disinterested witness).

Creates a statewide refusal registry maintained by the Department of Health Services to track residents who have refused to make anatomical gifts.

Requires the Department of Transportation to transfer information about residents who complete anatomical gift refusal forms when obtaining driver licenses to the refusal registry maintained by the Department of Health Services.

Prohibits the sale or purchase of body parts for transplantation or therapy for valuable consideration, with violations classified as a class 3 felony.
