Amends Arizona law to allow amendments to birth certificates for persons who have undergone gender transition or have a congenital intersex condition, replacing previous language limited to "sex change operation" or "chromosomal count."

Permits amendment based on either a physician's written statement verifying appropriate treatment for permanent gender transition or congenital intersex condition, or proof of legal name change with a statement that it was made to affirm gender identity without fraudulent intent.

Requires the state registrar to seal previously registered birth certificates and evidentiary documents after amendment, with access only through court order or as prescribed by rule.

Directs the Department of Health Services to establish a sliding fee scale for birth certificate copies and amendments for persons with annual income below 200 percent of federal poverty guidelines.

Exempts the Department of Health Services from standard rulemaking requirements for one year after the act's effective date to implement the fee scale.
