Adds a counseling exception to Arizona's parental consent requirement for minors seeking abortion, allowing abortion without parental consent if a physician or counselor provides specified pregnancy information and counseling.

Requires counseling to explain that information is objective and non-coercive, that the minor may withdraw her abortion decision at any time, and that alternatives to abortion exist including carrying to term and adoption.

Mandates counseling must explore all pregnancy management options with the minor including carrying to term with parenting or adoption, prenatal/postnatal care, and abortion procedures.

Requires the minor and counselor to sign a form documenting that counseling was provided covering alternatives, the right to change her decision, birth control information availability, and whether parental involvement was discussed.

Physician or counselor must keep documentation of counseling in the minor's medical records for seven years after the procedure or five years after the minor reaches adulthood, whichever is longer.
