Requires health insurance coverage for prosthetic and orthotic devices equivalent to Medicare coverage under Title XVIII of the Social Security Act.

Mandates coverage for device selection by treating physician, all services and supplies necessary for effective use, and medically necessary repairs or replacements to restore daily living or work activities.

Prohibits insurers from imposing different deductibles, copayments, coinsurance, benefit maximums, waiting periods, or lifetime/annual coverage limits on prosthetic and orthotic devices compared to other medical benefits.

Requires reimbursement rates for prosthetics and orthotics comparable to Medicare rates and to reimbursement for restorative internal devices under the same policy.

Applies to subscription contracts (HMOs), health maintenance organization evidence of coverage, disability insurance policies, and group or blanket disability insurance policies, but excludes limited benefit coverage.
