Prohibits persons under 18 years of age from using any tanning device in Arizona tanning facilities.

Requires tanning facility owners/operators to post notices about the age prohibition and health risks (skin cancer, premature aging, burns) and obtain signed customer acknowledgments before use.

Mandates tanning facilities provide free protective eyewear, maintain proper sanitation, limit exposure time per manufacturer recommendations, and restrict use to once per 24-hour period with interior temperature not exceeding 100°F.

Requires tanning facility owners to verify age with government-issued ID, maintain records of consent forms and customer exposure history for three years, and report any injuries to the Department of Health Services.

Establishes permit requirement for tanning facilities with annual expiration, civil penalties up to $100 for first violation and $300 for subsequent violations, and authority for the Department of Health Services to suspend or revoke permits for ongoing violations.
