Applies to the total amount billed across all related health care services from a single provider, whether in one or multiple bills.

Caps the health insurer's liability at the 150% Medicare rate for surprise out-of-network services covered by this provision.

Requires the Arizona Department of Health Services to adopt rules mandating nonprofit hospitals establish charity care policies for patients unable to pay, including income guidelines and fair hearing procedures.

Requires nonprofit hospitals to report annually on the amount of free or reduced-cost services provided; this reporting requirement repeals December 31, 2021.
