Amends Arizona's immunization reporting system to include adult immunizations in addition to child immunizations.

Requires health care professionals to report adult patient name, address, social security number (if not confidential), and date of birth along with vaccine type and administration date.

Allows the department to release adult immunization identifying information to the same entities currently authorized for child immunization records, including health care providers, schools, AHCCCS, and designated entities.

Maintains confidentiality protections and allows individuals to request that their immunization information be withheld from the reporting system.

Preserves existing penalties for non-compliance and unauthorized disclosure of immunization information.
