Amends Arizona Revised Statutes section 20-1095.06 to require service contracts to clearly indicate whether preexisting conditions are covered or excluded under the terms of the contract.

Removes previous language that allowed service contracts to exclude preexisting conditions only if conditions were known or would have been known by visual inspection, operation, or testing.

Requires service companies to submit all service contract forms to the director for approval at least 30 days before the proposed effective date.

Permits service companies holding existing permits to continue using previously approved service contract forms unless and until they submit a new form for approval after the effective date of this act.

Effective date: April 1, 2021.
