Amends Arizona Revised Statutes to add Article 9 to Title 14, Chapter 5, establishing a statutory framework for supported decision-making agreements for adults with disabilities.

Allows adults with disabilities to voluntarily enter into supported decision-making agreements with supporters who assist them in understanding options and consequences of life decisions without making decisions on their behalf.

Prohibits supporters from being surrogate decision-makers, signing legal documents for the adult, or receiving compensation for their role; supporters must act in good faith, with loyalty, and without self-interest or conflicts of interest.

Requires supported decision-making agreements to be signed by the adult and supporter in the presence of two witnesses or a notary public, and provides a statutory form for the agreement.

Agreement terminates upon written revocation by either party, when the adult becomes incapacitated, or upon appointment of a guardian; supporters who intimidate or deceive the adult are subject to criminal prosecution and civil penalties.
