Amends Arizona Revised Statutes sections 36-2601 and 36-2606 relating to the Arizona State Board of Pharmacy and controlled substances prescription monitoring program.

Adds new definition of "patient utilization report" to include controlled substances prescription monitoring program data and clinical alerts or other required alerts or indicators.

Requires medical practitioners to obtain a patient utilization report before prescribing opioid analgesics or benzodiazepines and requires dispensers to obtain a report before dispensing schedule II controlled substances, with limited exemptions for hospice, palliative care, cancer treatment, and acute injuries or procedures.

Directs the board to conduct annual voluntary surveys of program users to assess satisfaction and submit survey results to the President of the Senate, Speaker of the House, and Governor by December 1 each year.

Authorizes the board to promote data sharing agreements to integrate controlled substances prescription monitoring program data into electronic medical records systems.
