HB2621 Summary

• Requires the Arizona Department of Health Services to approve uniform prior authorization request forms for prescription drugs/devices/durable medical equipment and for all other health care procedures by January 1, 2022.

• Mandates that all providers use only the approved uniform forms and all health care services plans must accept them for prior authorization requests submitted on or after January 1, 2023; requests submitted on non-approved forms are invalid.

• Specifies that uniform prior authorization request forms must not exceed two printed pages (excluding supporting provider documentation) and must meet electronic submission and acceptance requirements.

• Requires the department to consider existing forms from the Centers for Medicare and Medicaid Services, national electronic prior authorization standards, and forms adopted by other state agencies, and to seek input from providers, health plans, utilization review agents, pharmacists, and pharmacy benefit managers.

• Permits payors to use internet webpages or web-based prior authorization systems that are consistent with the approved uniform forms.
