Adds diabetes outpatient self-management training as a covered service under Arizona's health care cost containment system (AHCCCS), limited to up to 10 program hours annually.

Covers diabetes training services when prescribed by a primary care practitioner for members newly diagnosed with diabetes or for previously diagnosed members experiencing a change in diagnosis, medical condition, treatment regimen, or not meeting appropriate clinical outcomes.

Specifies that monies from the hospital assessment fund cannot be used to fund the diabetes outpatient self-management training services.

Amends section 36-2907 of Arizona Revised Statutes to define diabetes outpatient self-management training consistent with federal Medicare definitions under 42 United States Code section 1395x.
