Appropriates $2,167,000 from the state general fund in each of fiscal years 2021-2022, 2022-2023, and 2023-2024 to the Arizona Health Care Cost Containment System for the healthy communities health care program, with emphasis on rural and urban underserved communities and tribal lands.

Requires the program to screen vulnerable persons for eligibility, manage insurance and enrollment, provide telehealth clinical care for chronic conditions and behavioral health, and employ trained life coaches to assist patients with barriers to health care.

Mandates development of a patient mobile application that complies with HIPAA and criminal justice privacy standards and enables secure communication between patients and providers, voluntary location sharing, and personalized treatment plans.

Funds community program operations, community managers, coach training, evidence-based programs, technology expenses, and outcomes reporting; clinical care delivery and coaching compensation must be provided through existing reimbursement mechanisms.

Requires quarterly reporting on job creation and patient outcomes to legislative committees and repeals the appropriation on December 31, 2024.
