Requires electors to make a verbal or signed request to receive an early ballot; request must be made within 93 days before an election and include name, address, date of birth, and state or country of birth.

Early ballot request forms distributed by candidates, political committees, or organizations must list the political subdivision as the return addressee; failure to do so results in a civil penalty up to three times the cost of production and distribution.

Persons, political committees, or organizations receiving completed early ballot request forms must submit them within six business days of receipt or eleven days before election day (whichever is earlier); failure to submit results in civil penalties of up to $25 per day per form and class 6 felony for knowing violations.

Prohibits election officers from delivering or mailing early ballots to any person who has not requested an early ballot for that election, except for voters on the active early voting list; violations constitute a class 5 felony.

County recorders must mail early ballots within five days of receipt from the ballot preparation officer, but not more than 27 days before the election; requests made 27 days or less before election must be mailed within 48 hours.
