Amends section 16-621 of the Arizona Revised Statutes relating to the conduct of elections and proceedings at counting centers.

Requires county recorders or officers in charge of elections to maintain records of all voting irregularities occurring during early voting, emergency voting, and election day voting, including descriptions of each irregularity, locations, persons present, and official responses.

Mandates that county election officials provide a written report of voting irregularities to the president of the Senate and speaker of the House of Representatives, along with a copy to the secretary of state, within thirty days after election day.

Requires live video recording with date and time indicators of all ballot custody in tabulation rooms at counting centers for statewide, county, or legislative elections, with recordings linked to the secretary of state's website and retained as public records for at least the challenge period.

Maintains existing chain of custody requirements for election equipment and ballots from early voting through completion of provisional voting tabulation.
