Allows dental hygienists to practice under an "affiliated practice relationship" with a dentist in specified settings (health care facilities, long-term care, public health agencies, schools, and government programs) without requiring the dentist's physical presence.

Requires dental hygienists in affiliated practice to have 500 hours of active practice in each of the two years before entering the relationship and maintain written agreements specifying services, procedures, standing orders, and consultation requirements.

Permits affiliated practice dental hygienists to perform most dental hygiene procedures except diagnostic procedures reserved for dentists and nitrous oxide administration, subject to conditions in the written agreement.

Removes the restriction limiting dentists to supervising no more than three affiliated practice dental hygienists at one time.

Requires both the dental hygienist and dentist to notify the Arizona State Board of Dental Examiners within 30 days of beginning or terminating an affiliated practice relationship and provide a copy of the written agreement.
