Department of Insurance shall annually establish minimum coverage rates and limits for adult and child hearing aids that provide at least 100% coverage of reasonable and customary hearing aids.

Department shall create an approved provider list every two years of licensed audiologists and hearing aid dispensers that health insurers must use for subscribers and enrollees.

All health insurance plans (subscription contracts, health maintenance organizations, disability policies, group disability policies, blanket disability policies, and health benefits plans) must provide hearing aid coverage per the department's established rates for each deaf or hard of hearing ear.

Physicians or licensed audiologists must document hearing loss in each ear to qualify for coverage.

Insured individuals may choose higher-priced hearing aids and pay the difference above the minimum coverage amount without financial or contractual penalties.
