Adds a new exception to Arizona's parental consent requirement for abortion on unemancipated minors: allows abortion without parental/guardian consent if a physician or counselor provides specific pregnancy information and counseling and the minor signs a form documenting receipt of that counseling.

Requires counseling to cover prenatal and postnatal care, alternatives to abortion (carrying to term, adoption, foster care), birth control information, and whether involving parents or guardians would be in the minor's best interests.

Mandates the counseling provider explain that the information is objective and non-coercive, that the minor may withdraw an abortion decision at any time, and provide opportunity for the minor to ask questions.

Requires the minor to sign a form confirming all counseling requirements were met, and the physician or counselor to sign, date, and maintain a copy in medical records.

Does not remove existing requirements for parental consent, judicial bypass procedures, or the 48-hour court hearing timeline for judicial authorization of abortion without parental consent.
