Requires written notarized parental consent from one parent, guardian, or conservator before performing an abortion on an unemancipated minor, with limited exceptions.

Allows a minor to petition superior court for authorization to bypass parental consent if the judge determines she is mature and capable of informed consent, or if the abortion would be in her best interests.

Adds a new exception allowing minors to consent to abortion independently if the attending physician determines the minor is mentally and physically competent and informs her of pregnancy status, duration, and associated risks.

Maintains existing exceptions for pregnancies resulting from sexual conduct by a family member or household resident, and for medical emergencies threatening the minor's life or causing serious bodily harm.

Establishes civil liability allowing parents or guardians to sue for damages including $5,000 statutory damages or three times the abortion cost (whichever is greater), with six-year statute of limitations; establishes criminal penalties for violations.
