Amends Arizona Revised Statutes section 36-2934.01 to modify requirements for trusts created under federal Social Security Act provisions that allow individuals to qualify for the Arizona Long-Term Care System (ALTCS).

Requires trust documents to name state Medicaid agencies as remainder beneficiaries entitled to receive their proportionate share of remaining trust assets equal to total medical assistance paid on behalf of the beneficiary upon trust termination or beneficiary death.

Adds new allowable trust disbursements including payments to ABLE accounts established under 26 U.S. Code section 529a and transfers between certain trusts established for the same beneficiary.

Clarifies that allowable payments may be made by direct payment from the trust or by payment of credit card receipts, with managed prepaid cards only permitted with appropriate restrictions.

Adds "or guardian" to the definition of financially responsible relatives who may provide personal care services to child beneficiaries under age eighteen, with payments limited to the administration's fee-for-service rate.
