Requires the director of the Arizona Department of Health Services to submit annual financial and programmatic reports on the state hospital by January 1 of each year to the governor and legislature, including revenues, expenditures, and administrative costs.

Mandates detailed breakdown of patient data at state hospital facilities including demographics (gender, race, age), legal status, county of origin, program type, diagnosis, and length of stay.

Requires reporting on civil commitments including admission and discharge numbers, wait times between admission requests and decisions, and reasons for admission denials.

Requires collection and reporting of safety plan data including number and types of assaults (with sexual assaults separately categorized), assaults reported to law enforcement or regulatory agencies, and law enforcement responses.

Establishes a joint legislative psychiatric hospital review council with eight members from various backgrounds to review and make recommendations on psychiatric hospital capacity, treatment programs, legal barriers, and related issues; council must submit reports by December 31, 2021 and 2022, with the section repealing August 31, 2023.

Directs the Department of Health Services to issue a request for information by October 1, 2021 for a surveillance system at the state hospital with audio and video capability and adequate file storage consistent with similar hospitals.
