Establishes a state program providing free blood testing for perfluorinated chemical exposure through private or public water supplies for persons meeting specified criteria.

Covers blood test costs when person's water supply exceeds drinking water criteria, was previously denied testing, or lives in a city/town with suspected or detected perfluorinated chemical releases.

Requires the department to assess and report to the public on health conditions associated with perfluorinated chemical exposure (kidney, liver, testicular cancer, low birth weight, miscarriages, ulcerative colitis, thyroid disease) when exposure exceeds 50 parts per trillion or the groundwater quality standard.

Appropriates $300,000 from the state general fund in fiscal year 2021-2022 to the Department of Health Services to implement the program.

Program terminates July 1, 2031, and becomes effective December 31, 2021.
