Requires written parental permission before students can participate in school student groups or clubs involving sexuality, gender, or gender identity.

Mandates school districts provide parents the opportunity to review formational documents (bylaws, mission statements, goals) of student groups or clubs related to sexuality, gender, or gender identity.

Requires school districts to notify parents in advance of and obtain written permission for any instruction, learning materials, or presentations regarding sexuality in courses other than formal sex education curricula.

Defines "formational documents" for purposes of parental review as including bylaws, statements of purpose, mission statements, goals, or related documents.

Applies parental consent and review requirements to all public educational institutions including school districts, charter schools, accommodation schools, and Arizona state schools for the deaf and blind.
