Requires hospital service corporations, medical service corporations, health care services organizations, disability insurers, and group/blanket disability insurers to provide coverage for biomarker testing in policies issued or renewed on or after January 1, 2023.

Mandates coverage for biomarker testing when used for diagnosis, treatment, management, or monitoring of disease or condition to guide treatment decisions, provided the test demonstrates clinical utility as supported by FDA approvals, CMS coverage determinations, or nationally recognized clinical practice guidelines.

Requires insurers to limit disruptions in care, including minimizing the need for multiple biopsies or biospecimen samples.

Establishes that subscribers, enrollees, and insureds must have access to a clear, readily accessible, and convenient online process to request exceptions to coverage policies, available on the insurer's website.

Applies the same requirements to the Arizona Health Care Cost Containment System (AHCCCS) administration and its contractors, with coverage provided at the same scope, duration, and frequency as other member benefits.
