Requires voters or their agents who deliver early ballots at polling places, voting centers, or emergency voting centers to present identification and provide written attestation to election board workers.

Specifies that voters delivering their own early ballots must present identification showing their name reasonably appears the same as on the ballot affidavit.

Specifies that a voter's agent delivering another person's early ballot must present identification and attest in writing that they are the voter's family member, household member, or caregiver as defined in statute.

Makes it a class 6 felony to knowingly violate the identification and attestation requirements for early ballot delivery.

Applies the attestation and identification requirements to all voting centers and emergency voting centers that accept voted early ballots.
