Requires hospital service corporations, medical service corporations, health care services organizations, and disability insurers to provide coverage for screening, genetic counseling, and testing for harmful mutations in the BRCA gene for subscribers and enrollees effective January 1, 2023.

Mandates that covered benefits be made available through health care providers who participate in the insurer's or organization's network plan.

Requires primary care providers to assess adult patients for personal or family history of breast, ovarian, tubal, or peritoneal cancer and BRCA gene risk factors using appropriate screening tools.

Directs primary care providers to provide genetic counseling and refer for or administer genetic testing when screening indicates a patient is at risk for harmful BRCA gene mutations.

Authorizes Arizona Department of Health Services and its contractors to provide BRCA screening, genetic counseling, and testing to members when clinically indicated, to the extent permitted by federal law.
