Creates youth music and art special license plates in Arizona, contingent on receipt of $32,000 implementation fee paid by a private party by December 31, 2022.

Establishes a youth music and art special plate fund receiving $8 per plate for administration and $17 per plate annually as donations from plate fees.

Requires fund monies to be allocated to a 501(c)(3) charitable organization located in Arizona that has existed since 1995 with a mission to serve youth ages 12-21 through music, dance, and art instruction.

Allows combination of youth music and art special plates with personalized plates subject to combined fees as determined by the director.

Amends multiple Arizona Revised Statutes sections to incorporate the new special plate category into existing license plate provisions and highway fund revenue definitions.
