Physicians licensed in Arizona must provide patients with written informed consent materials before performing breast implant surgery, including descriptions of risks, manufacturer patient information, and an informed consent checklist.

The informed consent checklist must include information on breast implant-associated anaplastic large cell lymphoma, breast implant illness, the national breast implant registry, and how to report adverse events to the FDA's MedWatch program.

The Arizona Medical Board and Arizona Board of Osteopathic Examiners must convene a work group with licensee representatives and patient advocates to develop and update the informed consent checklist.

Physicians who knowingly violate these informed consent requirements commit unprofessional conduct and are subject to disciplinary action.

"Breast implant surgery" is defined as surgical placement of a breast implant for cosmetic or reconstructive purposes.
