Prohibits the Arizona Department of Health Services from contracting with any entity that performs, promotes, or operates facilities where abortions are performed or conducted on or after the bill's effective date for the Personal Responsibility Education Program (42 U.S.C. § 713) or Sexual Risk Avoidance Education Grant Program (42 U.S.C. § 710).

Requires the Department to adopt rules mandating that applicants, grantees, and contractors complete attestations confirming abortions are not performed at their distinct locations and assign each location a distinct identification number as a condition of program eligibility.

Defines "abortion" using the same meaning prescribed in Arizona Revised Statutes section 36-2151.
