Declares any ballots not included in chain of custody documentation as invalid and prohibits their counting.

Makes it a class 2 misdemeanor for a person to knowingly place a ballot into collection, verification, or tabulation processes outside of the official chain of custody.

Allows voters to file civil action for damages against the governmental body administering an election if a provisional or early ballot is accidentally misplaced and cannot be counted, provided the ballot identifies the voter on the affidavit or envelope.
